In order to register your child for next year’s program, you will need to complete a new set of application forms. Registration forms can be found below. Please be sure to fill out the application completely.
- Every line on the Enrollment form must be filled out. Leaving blank spaces or writing “same as above” will delay your child’s enrollment.
- If your child does not have a dentist, please fill in that space with your child’s pediatrician’s name and address.
- Two pages ask for parents’ cell phones, please fill out both completely.
- On the emergency pick–up form, please be sure to put the full address as well as the phone number for each person whom you have listed.
A non-refundable registration and activity fee will be required at the time of registration. If you have a returning student, it is not necessary to get new health and immunization certificates unless you are notified that your current forms are expired.
